Order Your Destiny Report . . .
 
There are two choices available for the front cover graphic for your report. By default, I use the top one for reports for females and the lower one for males. You may choose either. Utilizing the link below, forward an email providing me with your name (as you would like for it to appear on the report) and your complete birth date. The time of birth is not required. If you have a preference for cover images, simply designate your choice in the email.

While the most expedient method for providing this information is through the email (in .pdf format), for portability, many prefer the bound and printed, version of this 30 page document. At checkout, you can designate which format you prefer.

If you are ordering multiple reports (for friends, children or other family members), please make sure that you have included the name and birth date information, as well as the choice of cover page formats.

**Note: Your "Life Lessons" information will accompany your Destiny Report.
